Waiver & Consent Form

This section ensures parents acknowledge risks, policies, and consent to participation.

1. Acknowledgment of Risk

• “I understand that soccer is a physical sport and acknowledge the inherent risks of injury, including but not limited to sprains, fractures, or concussions.”

2. Medical Authorization

• “I authorize Advance Soccer Training staff to administer basic first aid and, if necessary, seek emergency medical care in case of injury.”

3. Liability Release

• “I release Advance Soccer Training, its staff, coaches, and affiliates from liability for any injuries or damages that may occur during training, except in cases of gross negligence.”

4. Photo & Video Consent

• “I grant permission for photos and videos of my child to be used for promotional purposes, including social media and marketing materials.”

5. No Refund Policy Acknowledgment

• “I understand that all camp registrations are final, and no refunds will be issued under any circumstances.”
