TERMS & CONDITIONS

1. Services Provided

Criterion Medical PLLC (“Criterion Medical,” “we,” “us”) provides specialty medical care for substance use disorders via a Direct Patient Care (DPC) membership model. Our services include medication-assisted treatment (MAT), recovery planning, and care coordination.

2. Not an Emergency Service

IF YOU ARE EXPERIENCING A MEDICAL OR PSYCHIATRIC EMERGENCY, CALL 911 OR GO TO THE NEAREST EMERGENCY ROOM. Criterion Medical is not an emergency service and does not provide 24/7 crisis intervention.

3. Membership & Billing

  • DPC Model: We do not bill insurance. By joining, you agree to pay a monthly membership fee (Foundations at $275/mo or Stability at $185/mo).

  • Excluded Costs: Membership fees cover clinician visits and coordination. They do not cover the cost of medications, laboratory tests, or external therapy. You may use your insurance for these third-party costs.

  • Payments: Fees are billed monthly. Failure to pay may result in a suspension of services or refills.

4. Telehealth Consent

By using our virtual visit options, you acknowledge the risks and benefits of telemedicine, including potential technical limitations. You must be physically located in a state where our clinicians are licensed (New York/Hudson Valley region) at the time of the visit.

5. Communications (TCPA Compliance)

By providing your phone number and checking the consent box, you agree to receive non-marketing text messages (such as appointment reminders) from Criterion Medical.

6. Termination of Care

We reserve the right to terminate the physician-patient relationship for reasons including, but not limited to, non-payment, repeated missed appointments without notice, or behavior that threatens the safety of our staff.
