Financial Policy for Patients Enrolled in Payment Plans

This financial policy outlines the terms and conditions for patients opting for payment plans at Natura Integrative and Functional Medicine LLC.

By enrolling in a payment plan, you agree to the following:

1. Payment Terms:

Payment plans must be agreed upon and documented in writing before services are rendered.Payments will be charged automatically to the card on file on the agreed-upon dates.Failure to provide a valid card for auto-payment may result in the suspension of services.

2. Missed Payments:

If a payment is declined, the patient will be notified and must provide updated payment information within 3 business days.A late payment fee of $35 will apply for payments more than 5 days past due.

3. Default Payments:

Accounts more than 30 days overdue will be sent a final notice. After 60 days, the account may be referred to a collections agency. Services will be paused until the account is brought up to date.

4. Refunds and Cancellations:

Payment plans are non-refundable unless otherwise specified in your treatment agreement.If a patient cancels mid-treatment, the remaining balance must be paid in full.

5. Financial Hardship:

Patients experiencing financial hardship may contact our billing department to discuss alternate arrangements to pay off the remainder of the balance. Approval is at the clinic's discretion.

6. Responsibility Agreement:

By signing up for a payment plan below, you agree to adhere to these terms and ensure timely payments. Any deviations must be requested and approved in advance.
