Payment Agreement
This agreement is between: [customer.name], “the Client” and “the Company”.
I, [customer.name] am voluntarily entering into this agreement of my own free will. I have read and agree with, and shall comply with all terms of this agreement.
I agree that I am buying the [plan.name] membership plan in full now.
I, [customer.name] understand and agree that I am buying the [plan.name] membership for the total sum amount of all of the periodic, weekly, or monthly payments (specified by the selected plan), which the grand total for all payments is due and payable immediately upon executing this agreement and thus, purchasing my membership. This shall be completed before attending any further activities hosted by the Company.
The Company MAY opt to accept equal periodic, weekly, or monthly payments (specified by the selected plan) for the period of the membership (i.e. 6 or 12 months, whatever the period of the specified membership is).
I understand and agree that if I become delinquent by 10 days or more the Company MAY immediately charge the entire remaining balance to any of my payment accounts on file. I agree that the Company may immediately and repeatedly keep charging my accounts on file until the full amount is paid in full.
I agree that I will keep current on my payments and keep all payment information up to date, in order to facilitate the uninterrupted collection of my membership purchase total until the entire grand total of all payments has been paid to the Company in full. I further understand and agree that any and all fees and costs associated with collection of my account shall be my responsibility and I shall pay said fees to the Company immediately upon receipt or demand and said fees MAY be immediately charged to any of my payment accounts on file.
I understand and agree that my membership shall automatically renew for another membership period (6 or 12 month period, whichever plan was initially selected) on each anniversary date of my membership purchase until I take the required actions to cancel the membership.
To cancel or change my membership in any way, I understand and agree that I must make any and all requests in writing no less than 30 days prior to the affected billing cycle month, although 60 days is recommended.
I understand, accept, and agree that the Company may be unable to always provide consistent & scheduled services for the entire period of my membership and agree that I am not entitled to any refunds, compensation, discounts, or accommodations for any reason whatsoever.
Contract Start Date: Date & time that this digital document was digitally signed.
Contract End Date: No less than 30 days after official written notice of termination has been proven to have been received after the end of the prearranged commitment period has elapsed.
Contract Duration and Automatic Renewal: Contract is in force for 12, 6 or 1 months (depending on agreement selected) from the Contract Start Date, with automatic renewals for subsequent term(s), unless canceled by either party, with notification, sent via US Postal Service, IN WRITING no less than 30 days in advance of the affected billing cycle.
Agreement to Pay Recurring Fees: The Client agrees to pay weekly or monthly recurring fees via Client authorized automatic credit card. Payment in periodic, weekly or monthly increments by any means other than Client’s credit card or EFT is not permitted.

(a) I agree to purchase the [plan.name] for the entire total of weekly or monthly payments, as an automatic charge to my credit card account each period, week, or month for the contracted period of months with subsequent automatic renewals.
(b) I hereby certify that I am the holder of the credit card.
(c) I understand that I will be notified if my credit card payment fails to authorize for any reason, and that a $10 late fee will apply if I do not provide a valid credit card within 7 calendar days of the original rejection date.
(d) I understand that my service MAY be deactivated if my account becomes more than 30 calendar days late and that the entire remaining balance may be immediately charged to any of my payment accounts on file at that time.
This contract may be enforced at any time. Delay, action, or inaction on the Company’s part does not void or take away from the Company’s right and authority to enact or enforce any or all of the terms of this agreement at any time.
All sales are final. No Refunds.
Freeze your Auto Pay
The Company may or may not freeze your auto pay one time for up to 30 days upon request. There will be no freezes greater or less than 30 days. In order to opt to freeze your auto pay, your account must be current and in good standing, a completed request must be submitted in writing no less than 30 days in advance of the affected billing cycle month in order for your request to be effective for the following month. Billing date is based on your activation date listed below. If a freeze is allowed, any unused sessions may be distributed throughout the remainder of your membership at The Company’s convenience and discretion, as The Company deems appropriate.
Reservations, Cancellations & No Shows
Reservations are always required.
Reservations for morning sessions & events must be made no later than 9pm the night before the session or event start time. Reservations for day and evening sessions must be made no later than 2 hours prior to the session or event start time.
No shows & session cancellations made less than 2 hours prior to session start time will still be deducted from your allotment and considered used or spent and will not be refunded or credited.
Appointments and event cancellations must be canceled by the client no less than 24 hours prior to the appointment or event start time.
Late cancellations & no shows will be charged a no show fee.
I understand and agree that appointment credits, class credits, and other credits do not roll over from one week, month, or period to the next. If not used within the issue period, they are considered used or spent.
By attending classes, sessions, and or events, I agree to the terms and conditions.
