If you believe that material you uploaded or provided was removed or disabled by mistake or misidentification, you may submit a counter-notice to the Designated Copyright Agent containing:
Your physical or electronic signature.
Identification of the material that was removed or disabled and the location where it previously appeared.
A statement that you have a good faith belief the removal or disabling of the material was the result of a mistake or misidentification.
Your name, address, telephone number, and email address.
A statement that you consent to the jurisdiction of the appropriate federal court and that you agree to accept service of process from the person who filed the original infringement notice.
