We are required by law to:



Maintain the privacy of your Protected Health Information (PHI).

Provide you with this Notice of our legal duties and privacy practices.

Abide by the terms of the Notice currently in effect.

Notify you in the event of a breach of your unsecured PHI.

 

I. How We May Use and Disclose Your PHI (Without Your Authorization)

 

The law permits us to use and disclose your PHI for the purposes of Treatment, Payment, and Health Care Operations (TPO).

TreatmentWe may use and disclose your PHI to provide, coordinate, or manage your health care and any related services. Example: Sharing your medical history with a licensed provider for a telemedicine consultation, or with a compounding pharmacy to dispense your prescribed GLP-1 medication.B. PaymentWe may use and disclose your PHI to bill and get payment for the services we provide. Example: Sending a claim or invoice to you, your health insurance, or a third-party payer to obtain reimbursement for services rendered.C. Health Care OperationsWe may use and disclose your PHI for activities necessary to run our practice, such as quality assessment, auditing, and business management. Example: Reviewing patient records to evaluate the performance of our providers or engaging a Business Associate (like a secure electronic health record vendor) to manage your data.D. Public Health and SafetyWe may disclose your PHI to public health authorities for activities like preventing or controlling disease, reporting adverse reactions to medications (such as GLP-1s), or to prevent a serious threat to health or safety.E. Law Enforcement and Legal ProceedingsWe may disclose your PHI in response to a court order, subpoena, or other legal process as required by law.

 

 

II. Uses and Disclosures That Require Your Written Authorization

 

We must obtain your written authorization for the following uses and disclosures:

Marketing: Most uses of PHI for marketing purposes (though certain communications about treatment options or health-related products and services do not require authorization).

Sale of PHI: Disclosures that constitute a sale of PHI under HIPAA.

Other Uses: Any other use or disclosure of PHI not described in this Notice.

You may revoke an authorization in writing at any time, but the revocation will not apply to any actions we have already taken in reliance on the authorization.

 

III. Your Individual Rights Regarding Your PHI

 

You have the following rights concerning your medical information:

We are not generally required to agree to a requested restriction, except if the request is to a health plan for a service you paid for in full, out-of-pocket (as long as the disclosure is not otherwise required by law).Right to Confidential CommunicationsYou have the right to request that we communicate with you about medical matters in a certain way or at a certain location. Example: Requesting that we only call your cell phone or send mail to an alternative address. We will accommodate all reasonable requests.Right to Inspect and CopyYou have the right to inspect and obtain a copy of your PHI in our records (the Designated Record Set). We may charge a reasonable, cost-based fee for copying, mailing, or other supplies.Right to AmendIf you believe that the PHI we have about you is incorrect or incomplete, you may ask us to amend the information. We may deny your request, but we will provide you with a written explanation.Right to an Accounting of DisclosuresYou have the right to request a list of certain disclosures we have made of your PHI (excluding disclosures for TPO and those you specifically authorized).Right to a Paper Copy of this NoticeYou have the right to receive a paper copy of this Notice, even if you have agreed to receive it electronically.

 

 

IV. Changes to This Notice

 

We reserve the right to change the terms of this Notice at any time. The new Notice will be effective for all PHI we maintain. We will post a copy of the current Notice on our website  and in our facility.

 

V. Complaints

 

If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer at the contact information listed above. You may also file a complaint directly with the Secretary of the U.S. Department of Health and Human Services, Office for Civil Rights (OCR).

We will not retaliate against you for filing a complaint.
