Before you begin
To become a client at Revitalizing Pet Care, you'll need to select the care plan that fits your dog's needs.
Once you've enrolled in your care plan, return here to complete your intake forms and schedule your onboarding.
1. Complete your new client intake form
This form collects your basic information so we can set up your client and pet file in our system. It helps us get everything ready behind the scenes before your onboarding call.
2. Submit your dog's health history
Tell us about your dog’s past and present – diet, routines, symptoms, prior care, and anything else that helps us understand their whole picture. This information lets us prepare properly for your in-person visit and ensures nothing important gets missed.
3. Call to schedule your onboarding call
This is where we set the stage for your in-clinic appointment and personalized plan. We’ll go over your forms, ask questions, and schedule your in-clinic appointment.
