By providing us with your insurance details ahead of time, our billing team will conduct a thorough review of your specific plan. We’ll provide you with a clear summary of what is covered, helping you make informed decisions about your treatment plan without financial guesswork.

Understanding the Difference Between an EOB and a Bill

After your visit, we will submit a claim to your insurance carrier on your behalf. Most claims are processed within a week, though some carriers may take up to 90 days. Once your claim is processed, your insurance carrier will send you an Explanation of Benefits (EOB) outlining the charges, any adjustments, and your remaining patient responsibility — if any.

We receive the same information from your insurance carrier and use it to prepare your statement. If your insurance covers your visit in full, you won't receive a statement from us. If a balance remains, you will be billed.

To streamline your billing experience, Origins Integrative Health requires all patients to maintain at least one credit card on file. By providing your card information, you authorize Origins Integrative Health to charge any outstanding balance remaining after your insurance has been processed or payment is otherwise due. Your information is stored securely and will only be used in accordance with this agreement.

Missed & Cancelled Appointments

We understand that life happens — and we will always do our best to work with you. Out of respect for our practitioners and the patients waiting for care, we ask that you provide at least 24 business hours notice if you need to cancel or reschedule your appointment.

Please keep in mind that 24 business hours means 24 hours during clinic operating hours. If our office is closed over the weekend, Friday is the deadline to cancel a Monday appointment.

Appointments cancelled or rescheduled with adequate notice — by phone or text — will never incur a fee. However, failure to notify our office at least 24 business hours in advance will result in the following missed appointment fees:

  • First missed appointment: $175

  • Subsequent missed appointments: $100

These fees apply to both new and established patients and are not billable to insurance. We appreciate your understanding — honoring appointment times allows us to provide the highest level of care to every patient we serve.
